• Includes membership in Health Studio ordered by physician. .

• Requires a physical Therapist to design workout regimen w/activities in each session charted. .

• Requires that changes to regimen monitored by therapist and medical status will dictate review of regimen by physical therapist. .

• Outcome measures to be assessed quarterly.

• Hands-on care, of both a supportive and health-related nature, specific to the needs of a medically stable, physically handicapped individual. ,

• Supportive services are those which substitute for the absence, loss, diminution or impairment of a physical or cognitive function. This service may include skilled or nursing care to the extent permitted by state law.

Housekeeping activities which are incidental to the performance of care may also be furnished as part of this activity.

• Non-medical care, supervision and socialization provided to a functionally impaired adult. .

• Companions may assist or supervise the individual with such tasks as meal preparation, laundry and shopping, but do not perform these activities as discrete services. .

• Provision of companion services does not entail hands-on nursing care. .

• Providers may also perform light housekeeping tasks, which are incidental to the care and supervision of the individual. .

• Services are provided in accordance with a therapeutic goal in the Plan of Care, not divers ional in nature.

A homemaker does household chores that include housekeeping, cooking, shopping assistance, laundry, and other routine household activities. In addition to the above, a companion may provide companionship for the patient such as keeping a person company at home or going with the person to appointments or other outings.

These services include:

• daily living skills such as personal hygiene and grooming

• skills for household chores

• skills for meal preparation

• skills for shopping

• personal finances

• social skills

• Provides goal-oriented training and support to individuals residing in community and is intended to provide skills necessary to resume life after injury. .

• Services designed to foster successful community integration for individuals with TBI/SCI, and to lead a higher level of independent functioning. .

• Life skills support include: assistance and training with financial resources management, utilization of community resources and management of personal assistance services. .

• Training and support also includes areas related to activities of daily living:

• Provides assistance with eating, bathing, dressing, personal hygiene, activities of daily living. Services may include assistance with preparation of meals, but does NOT include cost of meals themselves. .

• Nutrition and food management to include purchasing, preparation, storage and serving of food. .

• Household management to include care of bedroom, bathroom, kitchen, care of clothing and safety in the home.

When specified in POC, service MAY also include housekeeping chores [bed making, dusting and vacuuming] incidental to the care furnished, or which are essential to the health and welfare of the individuals rather than the individual’s family.

• Coordinates community resources and natural supports. .

• Teaches new skills. .

• Assists with tasks and does things with or for the person. .

• Activities include assistance with the procurement of housing, financial management and banking, cooking, cleaning, shopping, personal care, personal relationships and community connections to mention few.

This service provides training and help to support the consumer to get and keep a job or run a small business. They help the consumer learn job skills or improve their skills so they can keep a job. They also help the consumer plan their goals including:

• type of work environment

• work activities

• hours of work

• level of pay

• the help needed to reach their goal
